Confidentiality & Non‑Disclosure Agreement

As a volunteer with Ascent 121, I understand that I may have access to confidential, sensitive, or personal information about clients, staff, or agency operations. I agree that:

- I will maintain strict confidentiality regarding all information I encounter.

- I will not disclose, repeat, copy, or discuss any identifying or sensitive information outside of my volunteer responsibilities.

- I will not photograph clients, client areas, or confidential materials.

- I understand that maintaining confidentiality is essential for client safety, privacy, and dignity.

I understand that violating confidentiality may result in immediate termination of my volunteer role and may carry legal consequences.

Liability Waiver

I understand that volunteering with Ascent 121 may involve participation in activities where injury or accident could occur. In consideration of being allowed to volunteer, I hereby:

- Release and hold harmless Ascent 121, its staff, board, volunteers, and partners from any liability, claims, or demands arising from participation in volunteer activities.

- Acknowledge that I am voluntarily participating and assume all risks associated with volunteer activities.

- Agree that Ascent 121 is not responsible for lost, stolen, or damaged personal property.

Medical Emergency Consent

- I authorize Ascent 121 staff or representatives to contact emergency medical services (EMS/paramedics) on my behalf in the event of an accident, illness, or medical emergency.

- I understand that Ascent 121 is not responsible for the cost of emergency treatment or transportation.

Background Check Notice & Consent

- A criminal history check is required before volunteering.

- I authorize Ascent 121 to conduct a background check through an approved third‑party service.

- I understand that final acceptance as a volunteer is conditional upon background check results.
